To request your personal information:
- Download and complete the
personal information form (PDF 95KB) - We also need proof of your identity. Please include the following with your application:
- Photographic identification (such as your current driving licence or any photo ID that includes your name and address)
- Proof of your current address (such as a copy of a recent utility or Council Tax bill)
- Email the form and proof of identity to foi@dacorum.gov.uk.
